Questions/Concerns - If you are experiencing an emergency, or do not feel like you can keep yourself or others safe, do not contact the office. Report immediately to the nearest emergency department and/or call 911. The fastest way to reach your provider is by sending a message via your patient portal. They can send you a response back that is encrypted and secure. You will receive an email when they have messaged you back. 

Requesting refills - The easiest and fastest way to request refills is through your patient portal.  There are issues frequently when requesting refills through the pharmacy, and the patient portal is very reliable and easy to use. In some instances, you will need an appointment in order for additional refills to be sent. 

Scheduling - Future appointments are typically made with your provider at the end of each session. It is possible, however, to cancel appointments and schedule new appointments via your patient portal. You may also contact the office at (402)-769-0880.

Confidentiality - Unless there is an imminent & life-threatening emergency, we do not share information without your written consent. Your other providers can fax their own consent forms to our office at: (402)-913-3128. You may also upload these forms onto your patient portal.

Cancellation and "no-show" policy - These can be found in your patient portal within the consent documents that you have signed.
